or if a person is Intellectually Disabled

Qualified Examiner wants to Register     The advantage of a Qualified Examiner registering with the DHS FOID Mental Health Reporting System is that if they return to make future reports they do not have to re-enter their demographic information each time. During this registration session, a Qualified Examiner can also report information, but does not have to.


Qualified Examiner does not want to register but wants to report information     A Qualified Examiner who chooses not to register with the DHS FOID Mental Health Reporting System will be required to enter their demographic information each time they report information.
